ISTOP update with Healthix Concise. See Attached.
You can access the istop directly from the patient's chart. Click on "Healthix concise" in the chart review under encounters. Scroll until you see the istop login
Screening
Screeners that were completed for the current visit can now been seen in the right-sided panel where you note is under "This Visit." See attached.
Getting to the mother's record for infants.
Hover over the Legal Guardian section.
You can then click on the mother and go directly to mom's chart for example to look at perinatal data and vaccine data example RSV vaccine data to see if the infant is eligible for the antibody.

Older adults/Senior Services/Homebound care
Integrity Senior Services provide in-home mental health counseling, case management, and even supportive cleaning services. They extend their services to those people who can benefit from counseling but are unable to leave home.

Friendly Calls for Seniors, Friendly Visiting is for traditionally homebound older adults, who have ongoing health challenges that make it difficult for them to go out. Volunteers visit the older adult in their home, on the phone, or on video to talk about shared interests and experiences, forming friendships in the process. You or the adult who wishes to join the program can call Aging Connect at 212-NYC-AGING (212-244-6469).

Jasa: They provide health and mental health services, home-delivered meals, social programming at senior centers, caregiver assistance and more.

Riverstone Senior Life Services: It's a Senior Center and they also provide Social Work services. They only provide in person services but they may provide transportation services.
