• Fax Rx to 631-753-3910, 

  • Rx must include:

    • Patient's name & DOB

    • Labs needed with associated Dx code(s)

    • Date of services needed for the lab draw (can make a standing order for up to 6m)

  • Also include in the faxed packet

    • Pt's insurance information (can copy and paste from chart) 

    • Providers name, NPI, Address, phone and fax

• To review results you need to create a Northwell health account
