Adolescent Mychart
Dear Registration & Access Team Staff,
To optimize adolescent access to the Connect patient portal and prepare for sharing adolescent clinical notes, the NY Consortium has begun an initiative to improve adolescent and proxy Connect portal enrollment. A critical aspect of this initiative is a standard training course for all staff which reviews the workflow for Connect patient portal activation and proxy assignment.
NY state law encourages patients between the age of 12 to 17-years-old to be the primary holders of their portal accounts. This allows adolescents to share more information with their healthcare providers while also empowering younger patients to learn more about their health.
Moving forward for patients ages 12 to 17-years-old, Connect accounts should be registered with the adolescent’s personal email address as the username and primary point of contact and should not be registered with a parent or legal guardian's email. When registering any patient 12 to 17-years-old, please verify that the patient's Connect account is registered under the adolescent's email. If this is not the case, the account should be deactivated and reactivated with the appropriate adolescent information.
For most adolescent patients 12 to 17-years-old registered in Connect, a "Parent to Adolescent" proxy account should be created for a parent or legal guardian. This provides the parent or guardian access to an adolescent's account while maintaining the confidentiality of sensitive information for adolescent patients.
In the next few days, you may receive an email regarding a training course that reviews the standard portal enrollment workflow for adolescents and proxies. This training documentation and video are also provided below with a list of frequently asked questions we expect patients, parents, and guardians to have regarding the process. Please review these educational materials as soon as possible.

Resident Prescriptions for Patients with NYS Medicaid
In summary: Resident and fellow prescriptions might be rejected from the pharmacy with the code 889 <Provider Not Enrolled in State Medicaid Program>. This is occurring because the New York State Department of Health (NYSDOH) has implemented a policy which requires all Medicaid Managed Care in-network and out-of-network furnishing, ordering, prescribing, and referring providers to enroll as a qualified Medicaid provider in the NYS Medicaid Program with an exception for resident/fellow prescriptions.
IT has created a smartphrase .medicaidrx which can be placed in the notes to pharmacy box in any prescription to help pharmacies understand the requirement and how to process the prescriptions.
